2. Medication Errors
  • Discuss the Essential Safety Requirements.
  • Determine the Role of pharmacist in managing the ESR.
  • Discuss the safety parameters for the ESR.
  • Learn how to establish ESRs throughout the MM.
3. Adverse Drug Reaction & Pharmacovigilance
  • Define the Adverse Drug Reactions(ADR), and Severity of ADR.
  • Discuss the reporting process for the ADR.
  • Illustrate Naranjo Causality Scale, ranks and scoring system.
  • Define the WHO assessment, and Upsala Monitoring assessment.
  • Determine the importance of reporting the ADR either in the national or the international levels.
  • Define the pharmacovigilance (PV), and its important.
