2. Medication Errors
- Discuss the Essential Safety Requirements.
- Determine the Role of pharmacist in managing the ESR.
- Discuss the safety parameters for the ESR.
- Learn how to establish ESRs throughout the MM.
3. Adverse Drug Reaction & Pharmacovigilance
- Define the Adverse Drug Reactions(ADR), and Severity of ADR.
- Discuss the reporting process for the ADR.
- Illustrate Naranjo Causality Scale, ranks and scoring system.
- Define the WHO assessment, and Upsala Monitoring assessment.
- Determine the importance of reporting the ADR either in the national or the international levels.
- Define the pharmacovigilance (PV), and its important.
- Determine the national and international PV organizations worldwide.
- Discuss the important resources of PV that help the In clinical practice.
4. Role of Pharmacist in Committee Meetings
