Scenario 1
- A 20–year–old man was diagnosed with generalized anxiety disorder when he was 15 years old. The patient’s plan of therapy included cognitive behavioural therapy. plus, he is taking Prozac (fluoxetine) 20mg
once daily. He has complained about his concern about everything in his life; he feels that the
medicine doesn’t control his Anxiety. The patient came to the pharmacist seeking advice. Patient Education Examples
Counselling:
- First, I asked the patient’s name and how he felt about his Anxiety. I am trying to build a trusting bond with him so he can feel comfortable talking to me.
- Chronic Anxiety means that you persistently worry about things in your life that would affect your daily functions negatively. For that reason, your doctor put you on some behavioural therapy and medicine so your life would be easily controlled.
- How do you feel at the moment?
- What do you make you feel that your medicine doesn’t control your Anxiety?
- Since when do you feel that your medicine lost its effect?
- Do you drink Alcohol?
- Do you experience any pain, fatigue, or shortness of breath? And if so, how many times a day?
- Do you have any medical condition other than anxiety, such as diabetes or any cardiac
disease? Are you on some chronic medicine? - Do you take any herbal or other medicine such as any painkiller or vitamins?
- Does anything new happen currently in your life?
- For how long do you take the Prozac? And how many times do you take it a day?
- Did you experience any side effects, such as impaired concentration or confusion?
- Do you usually miss your dose? If so, how many times do you skip your dose?
- Since you don’t feel that medicine affects you. Did you take more than one tablet a day or
when needed? Because if you do. I must inform you to take one dose a day as told by your doctor. - Plus, this medicine doesn’t have an immediate effect at the moment.
- I must advise you to speak to your doctor about your concerns. Patient Education Examples
My recommendation to the doctor
- G.A.D. re–assessment. Patient assessment is required (including lab tests, physical examinations, and mental status
examinations), the family history and psychiatric history. - Prozac dose titration in 1–2 weeks once the decision is made about the patient’s step up.
Scenario 2
- Patient name: A.B.S.
- Age: 60 years old
- Weight:84 kg
- Diagnosis: Hypertension
Medications
- Amlor (amlodipine )10 mg once a day
- Thiazide 25 mg once a day
- Ivarin (rosuvastatin) 20mg once a day
- Aspirin 75mg once a day
Counselling
- First, discuss with the patient about his condition and how he feels about it.
- You need to know that your hypertension means that your blood pressure is high all the time, and you need your medications daily to control it. It is a chronic disease not curable, but your medicine will help you avoid any further complications and make your life more flexible. Patient Education Examples
- All your medicine will be taken daily but at different times so it can be convenient for you.
- Take one tablet of Aspirin after breakfast every day. It will help you in preventing blood cults.
- Take one tablet of major after lunch daily. This medicine helps lower your blood pressure. Don’t take more than one tablet because, in your case, one tablet is adequate for your stress. If you miss a dose, take it as soon as possible.
- Please discuss with your doctor when you experience ankle oedema, as it is one of the side effects of the major.
- Thiazide is a diuretic that increases your urination (you need to get toilets) to decrease the pressure on your heart. So it’s better to take it when you are awake and at home. One tablet to take in the morning or in the afternoon. Patient Education Examples
- Ivarin 20 mg is for your cholesterol level, which reduces your cholesterol level to improve your blood pressure and prevent any further heart complications. Don’t double the dose. Take the tablet after dinner.
- You might experience some muscle pain with the ivarin. It is one of the side effects you discuss with your doctor. It might also cause some sleep disturbance, so avoid taking it before sleep.
- To improve your blood pressure, your medications won’t be enough alone. Do some exercise. Try to lose some weight. Avoid smoking. Have a healthy diet. Patient Education Examples
Scenario 3
- Name: Ahmed Mustafa
- Age: 32 years old
- Gender: male
- Weight: 70 mg
- Diagnosis: severe peptic ulcer
Medications:
- Nexium (esomeprazole) 40 mg twice a day for five days
- Clarity (clarithromycin) 500mg, one tab every 12 hours for five days
- Flazol (metronidazole ) 500mg, one tab every 12 hours for five days
- Panadol advanced ( paracetamol ) 500 mg one tab every 12 hours. When needed.
Counselling:
- First, you need to know that your condition makes you feel burned and have pain in your stomach due to the high acidity inside.
- Your Nexium will make you feel better, but you must take one hour before eating anything. Take one tablet before breakfast and one tablet before dinner for five days. The burning sensation will take minutes to disappear.
- Panadol advance if gentle on your stomach. It will release the pain. So take one tablet after a meal every 8 hours when you are in pain.
- If your pain doesn’t relieve, please do NOT take any other painkiller that you can buy from community pharmacies until you discuss it with your doctor first because some anaesthetic, such as diclofenac, worsens your ulcer more.
- You have two antibiotics, Claritt and Flazol. Both must take them as one tablet every 12 hours after meals. For example, if you take Claritt at 8:00 am and Flazol at 11:00 am, then you must take the next dose of Claritt at 8:00 p.m. and Flazol at 11:00 p.m. The course is for five days and takes more or less than five days. Don’t double the dose, as doubling the amount doesn’t mean that it will have a faster or better effect.
- You might experience some metallic taste when using flazol. It’s one of the side effects.
- Don’t drink any alcohol within and after the course for three days minimum.
- Inform your doctor before taking medication if you are on other medicines such as warfarin
- One, you miss a dose more than an hour. Inform your doctor, and you will start the course from the beginning.
- Avoid smoking and drinking caffeine during the treatment course. Patient Education Examples
Scenario 4
- Name: Manal Ahmed
- Age: 18 years old
- Gender: Female
- Weight: 55 kg
- Diagnosis: infected dermatitis on the neck
Medications:
- Fusiderm cream ( Fuidic Acid 20m) To be applied three times a day for five days
- Meloderm ointment (Beta-Sitosterol 0.25%) To use twice a day for five days
Counselling:
- You are diagnosed with an infected dermatitis that makes your skin swollen and sore due to bacterial infection. It will be treated once you take your medications.
- First: Before applying any cream or topical supply, make sure that you clean your skin and dry it clearly.
- And wash your hands.
- Fusiderm cream is an antibiotic that you must be sure to apply every 8 hours. Put a small amount on your finger to apply a thin layer to the affected area. Patient Education Examples
- Do Not apply it on an open wound.
- Be careful not to use it near your eyes.
- Use it for five days. Do not take too long. But when you feel that the cream doesn’t relieve your inflammation, call YOUR DOCTOR.
- If you experience any irritation or rash when using the products, call your doctor.
- Meloderm ointment (Beta-Sitosterol 0.25%)
- This ointment is herbal that helps you to reduce your inflammation and accelerate your skin healing.
- Apply twice a day, one time in the morning and one time at night.
- If you are allergic to any herb such as semsemi, tell your doctor before use.
- Both medications are stored at room temperature 25C.
