1 – Development, implementation, and evaluation of a health outcomes and research program at an integrated health-system specialty pharmacy

• Health-system specialty pharmacies (HSSPs) provide high-quality, efficient, and collaborative care to patients receiving specialty therapy.
• Despite proven benefits of the integrated model, manufacturer and payer restrictions challenge the viability and utility of HSSPs.
• Vanderbilt Specialty Pharmacy developed a health outcomes and research program to measure and communicate the value of this model, drive improvement in patient care delivery, and advocate for recognition of HSSP pharmacists’ role in patient care.
• The purpose of this descriptive report is to describe the development and results of this program..

2 – Precision medicine: Opportunities for health-system pharmacists

• Pharmacogenomics has been used as a broader term in reference to any genetic factor that impacts drug response, including somatic mutations in cancer, the role of pathogen genomes in infectious diseases, etc.
• Precision medicine has been defined by the National Research Council as “the tailoring of medical treatment to the individual characteristics of each patient.”.
• In practical terms today, precision medicine should be considered any care that is guided by individual patient characteristics, including genetic characteristics and other factors such as diet and lifestyle, that maximize treatment benefits .

• During the coronavirus lockdown it’s becoming harder and harder to maintain a work-life balance, or find time for relaxation whilst staying informed about what’s going on around us.
• Burnout is when you lose all motivation and enthusiasm for your work and you feel high levels of stress, anxiety, depression, overwhelm or even hopelessness or dread

5 – Review of Prothrombin Complex Concentrates Use in Apixaban and Rivaroxaban Associated Intracranial Hemorrhages

• This study aims to summarize the studies evaluating the use of 4-factor prothrombin complex concentrates in the management of apixaban and rivaroxaban associated intracranial hemorrhages.
• Rates of hemostatic effectiveness were influenced by the definition of hemostatic effectiveness, dose administered, and patient severity.
• Studies suggest that higher doses may result in higher hemostatic effectiveness rates without increasing the risk of experiencing a thromboembolic event.
• This review may be used by providers to modify or validate their reversal strategy approach until well designed studies are available

6 – Turning differentiation into value: How health-system specialty pharmacies can align to an outcomes-focused mission

• Despite the known differentiators of the health-system specialty pharmacy (HSSP) model such as proximity to provider, interdisciplinary team participation, and electronic medical record (EMR) access, the translation to value for external stakeholders continues to be underrealized.
• These challenges are partially drawn from the variable level of maturity of system-based operations but also, more importantly, from lack of defined outcomes measures for specialty therapies.
• To change the narrative surrounding medication access and inclusion in third-party payer contracts, the HSSP model needs leadership.
• In this case, leadership must come from a panel of thought leaders from mature programs..
