 1 – Predictive model for atrial fibrillation in hypertensive diabetic patients

  • Several scores to identify patients at high risk of suffering atrial fibrillation have been developed.
  • Their applicability in hypertensive diabetic patients, however, remains uncertain.
  • The aim of this study is to develop and validate a diagnostic predictive model to calculate the risk of developing atrial fibrillation at five years in a hypertensive diabetic population.
  • The model accurately predicts future atrial fibrillation in a population with both diabetes and hypertension.
  • Early detection allows the prevention of possible complications arising from this disease.

2- The impact of antiplatelet and antithrombotic regimen after TAVI

  • This study compared the outcomes and adverse events of TAVI patients based on the discharge and long-term antiplatelet or anticoagulant treatment regimens
  • The lower all-cause mortality for DAPT within the first year and N-VKAs over VKA within the first 3 years warrant considerable attention in further recommendations of antithrombotic and anticoagulation regimens after TAVI.

3- Performance of early risk assessment tools to predict the later development of gestational diabetes

  • Several prognostic models for gestational diabetes mellitus (GDM) are provided in the literature; however, their clinical significance has not been thoroughly evaluated, especially with regard to application at early gestation and in accordance with the most recent diagnostic criteria.
  • This external validation study aimed to assess the predictive accuracy of published risk estimation models for the later development of GDM at early pregnancy.
  • Most prediction models showed acceptable accuracy in terms of discrimination but lacked in calibration, which was strongly dependent on study settings.
  • Simple biochemical variables such as fasting glucose, HbA1c and triglycerides can improve risk prediction.

4- Metronidazole-associated Neurologic Events: A Nested Case-control Study

  • Case reports have described instances of peripheral and central nervous system toxicity during treatment with metronidazole; however, no large-scale studies have examined this association.
  • Metronidazole is associated with an increased risk of adverse peripheral and central nervous system events relative to clindamycin.
  • Clinicians and patients should be aware of these rare but potentially serious adverse events.

5- A Real-world Evaluation of a Case-based Reasoning Algorithm to Support Antimicrobial Prescribing Decisions in Acute Care

  • A locally developed case-based reasoning (CBR) algorithm, designed to augment antimicrobial prescribing in secondary care was evaluated.
  • A CBR-driven decision support system provided appropriate recommendations within a narrower spectrum compared to current clinical practice.
  • Future work must investigate the impact of this intervention on prescribing behaviors more broadly and patient outcomes.

6- Risk Factors and Outcomes of Hospitalized Patients With Severe Coronavirus Disease 2019 (COVID-19) and Secondary Bloodstream Infections: A Multicenter Case-Control Study

  • Coronavirus disease 2019 (COVID-19) has become a global pandemic.
  • Clinical characteristics regarding secondary infections in patients with COVID-19 have been reported, but detailed microbiology, risk factors, and outcomes of secondary bloodstream infections (sBSIs) in patients with severe COVID-19 have not been well described.
  • They observed that hospitalized adult patients with severe COVID-19 and sBSI had a more severe initial presentation, prolonged hospital course, and worse clinical outcomes.
  • To maintain antimicrobial stewardship principles, further prospective studies are necessary to better characterize risk factors and prediction modeling to better understand when to suspect and empirically treat for sBSIs in severe COVID-19.
